Request to Schedule Courses
Instructors may reserve bays with a deposit
All instructors need to provide range with proof of insurance with range added as ‘Additional Insured’
Final payments must be made prior to start of class
All students & instructors must have ETTS waiver on file. Please email course roster at least 1 week prior to course date.
Quotes to be sent upon completion of this form
Course Information

-Course date
-Amount of shooters/students
-Type of course
-If setup is being requested and specifics needed
-Special requests
